I voluntarily choose to receive a Cluster Lash application at Fairy Lashes & Brow.

I understand and acknowledge that:

1. Cluster Lashes are NOT the same as Professional Eyelash Extensions.
   Professional eyelash extensions are applied by attaching one extension (or a handmade fan) to one individual natural lash using professional isolation techniques.
   
   Cluster lashes consist of multiple lashes grouped together and attached across multiple natural lashes. This is considered a temporary cosmetic enhancement and is not intended to be worn like professional eyelash extensions.
2. Because multiple natural lashes may be attached together, cluster lashes may place additional stress on my natural lashes as they grow at different rates.
3. Cluster lashes are intended for short-term wear only. I understand they should generally be removed within 5–7 days, or sooner if recommended by my technician.
4. Keeping cluster lashes on longer than recommended may increase the risk of premature natural lash shedding, lash breakage, tangling, discomfort, irritation, and damage caused by improper removal.
5. I understand that Fairy Lashes & Brow cannot control my home care, sleeping habits, makeup use, rubbing of the eyes, exposure to water, oils, or how long I choose to wear the cluster lashes.
6. I agree not to pull, peel, or remove the cluster lashes myself. Improper removal may result in damage to my natural lashes.

I understand that all cosmetic services involve potential risks, including but not limited to:

• Eye irritation
• Allergic reaction
• Redness
• Watering eyes
• Natural lash shedding
• Temporary discomfort
• Damage resulting from improper aftercare or extended wear

I voluntarily accept these known and unforeseen risks.

To the fullest extent permitted by law, I release, discharge, and hold harmless Fairy Lashes & Brow, its owner, employees, technicians, contractors, and representatives from any and all claims, liabilities, damages, losses, costs, or expenses arising out of or related to my Cluster Lash service, including but not limited to:

• Damage to my natural lashes
• Loss of natural lashes
• Irritation or allergic reactions
• Dissatisfaction with longevity
• Damage caused by extended wear
• Damage caused by self-removal or removal by another person
• Failure to follow aftercare instructions

I understand that no guarantee has been made regarding the longevity or condition of my natural lashes after receiving this service.

I acknowledge that cosmetic services are non-refundable once completed.

If I experience concerns following my appointment, I agree to contact Fairy Lashes & Brow promptly so that my technician may evaluate the situation.

• I fully understand the differences between Cluster Lashes and Professional Eyelash Extensions.
• I have had the opportunity to ask questions.
• My questions have been answered to my satisfaction.
• I voluntarily consent to receive this service.
